What You'll Do:
- Execute incoming calls, assess client/patient needs, and
identify appropriate comprehensive and individualized services to
serve each client/patient best.
- Collect appropriate documentation to support and determine
eligibility for program services.
- Assist client/patient with scheduling dates for appointments or
classes.
- Process patient referrals for a variety of services, including
medical and dental, as well as programs, including behavioral
health.
- Monitor and document active referrals and client
participation.
- Assist patients in completing applications and forms required
by the program(s) in which they will participate. Ensure patient
information is complete and entered into the appropriate data
system.
- Respond to basic questions from patients.
- Contact the patient for follow-up regarding recommended
services and referrals. Encourage client participation and
retention in various programs and classes.
- Perform work to align with productivity goals and standards,
including # patients, # encounters, # appointments scheduled, and #
classes facilitated.
- Provide program administrative support such as data entry,
client file management, report development, and contract
compliance.
- Coordinate and promote YVFWC programs. Recruit clients/patients
for dental services, medical services, social services, or
education classes. Reserve conference rooms, obtain supplies, and
organize snacks for classes.
- Present program information in various internal and external
community meetings and functions as requested, perform other duties
as assigned
- This position includes home visiting as part of the essential
functions
- Tomando Master Trainer duties (if assigned): Required to train
and support CDSMP Lay Leaders, ensure the fidelity of the
curriculum and keep up to date with program changes, must maintain
Tomando Master Trainer Certificate.
Qualifications:
- High School Diploma or General Education Diploma (GED)
- One year of administrative support and customer service
experience required ; Two years' administrative support experience
in a social services or healthcare setting preferred
- Applicable license or certification ( some positions may
require program-specific certification)
- Tomando Master Trainer certification (if assigned Master
Trainer duties)
- Valid Driver's License and proof of automobile liability
insurance coverage
Skills:
- Bilingual (English/Spanish) preferred at level 9
- Skilled in planning, coordinating, and organizing work
projects
- Proficient in delivering professional written and verbal
presentations
- Capable of prioritizing tasks and managing multiple
responsibilities
- Excellent at interacting with the public, patients, and staff
professionally
- Knowledgeable in understanding and applying program
requirements and relevant regulations
- Knowledge of the social and health agencies in the area and the
services they provide
- Effective verbal, written, and listening communication
skills
- Basic proficiency with computer programs, including Microsoft
Office, Word, and Excel
